EHR Acknowledgement Form Example

Here is an example of how the EHR Form should be filled out. 


  • Please assure that ALL areas that are identified as Required are completely filled out. 
  • Staff must sign the form in wet ink. (not printed or typed)
  • Printed name must be legible. 
  • It helps if the name is typed in the body of the request to avoid misspellings
