PROBLEM: Modification Assessment Rejected Due to Section X0800

Modification assessment was rejected due to section X0800 being inconsistent Correct the modification and change the response for X0800 to the correct number.

 NOTE: Coding Instruction from the RAI Manual advises for the first correction request (modification / inactivation) of MDS record, code a value of 01 (zero - 1); for the second request code a value of 02 (zero - two); with each succeeding request, X0800 is incremented by one.  Values between one and nine should have a leading zero in the first box.

 

 TIP: It is not the number of answers within the MDS that you change but the number of times that the Modification has been sent and accepted. 

 Cause

If the modification is the first modification of the MDS assessment being sent to CMS, then section X0800 should be 1. Subsequent modifications submitted will need to be incremented by 1. If the modification is submitted a 2nd time, then this value is incremented to 2 and so on.

Inconsistent X0800 Value: The submitted modification value submitted in X0800 is not incremented by one 1 from the previously submitted modification value for this record.

Instructions:

To correct the modification, navigate to:

1. Clinical > Resident > MDS

2. Find the MDS that needs to be corrected > Click on the unlock link > Unlock the assessment

3. Go to Section X > X0800 > Un-Sign the Question

4. Change the response to the correct number > Sign > Complete and lock the assessment

5. Add to batch and resend to CMS.

 

Special Notes:

Since Center's Health Care utilizes automatic submission, we may receive a rejection with this warning if the submission was sent to CMS more than once.  You will not be able to see this on the front end as it is in the QIES ASAP system. 

The MDS coordinator/Accessor at the facility will have to contact the State RAI coordinator to find the total number of times it was submitted to the CMS system which may be more than the number of times the facility sent it in a batch The question is not asking how many times the facility sent it/pushed the submit button or the number of modification books created but is actually referring to what is going on in the background that the end user cannot see. The State RAI coordinator has the correct access to the qies asap system that would allow them to count the number of times the batch was submitted. 

 This is because the batch will continue to submit in the background if there is any interruption in the network including change of IP address and any other reason that could cause network connectivity issues or interruptions.

 If it is determined that the MDS was accepted on their validation report then they will clear the error and mark it as accepted. PCC will notify the EHR team and we will communicate to the facility what to do next or if the issue was resolved.


Coding Instructions for X0800, Correction Number from RAI Manual

 • Enter the total number of correction requests to modify/inactivate the record in the QIES ASAP system that is in error. Include the present modification/inactivation request in this number.

 • For the first correction request (modification/inactivation) for an MDS record, code a value of 01 (zero-one); for the second correction request, code a value of 02 (zero-two); etc. With each succeeding request, X0800 is incremented by one. For values between one and nine, a leading zero should be used in the first box. For example, enter “01” into the two boxes for X0800. 

• This item identifies the total number of correction requests following the original assessment or tracking record, including the present request. Note that Item X0800 is used to track successive correction requests in the QIES ASAP system.
