MDS Manual Submission from PCC to Simple LTC


Follow these steps to submit completed MDS assessments from PCC to CMS through Simple LTC

 

Part 1. PCC: Batch Creation 

  1. Clinical>Care Management>MDS>3.0 Batches. Click on New

 

 

  1. Add assessments that need to be submitted to the batch. 
  2. Click on Export

 

  1. A new window will open asking you if you want to save the files. Click Save. 

 

 

 

  1. Choose the location on your computer where you want to save the file. (you will need to remember this in part 2)

 

 

 

 

 

 

 

Part 2 Simple LTC: CMS Submission

Login to Simple LTC and complete the following steps.

  1. Click on MDS Tab
  2. Click on Upload tab.
  3. Click on Choose file
    1. A new window will pop out asking which file like when you choose to export the file from PCC. Choose the file you downloaded previously in order to upload it here.
  4. Click on the green submit to CMS button

 

  1. Monitor the progress on the MDS Transmission Activity Page

 

Part 3 PCC: Record Validation Report Results in PCC

Once the CMS Final Validation report is received in Simple LTC go back to PCC Clinical>Care Management>MDS Portal to view the 3.0 batches.

  1. Click Record Results on the Batch from the validation report

  1. Enter the Final Validation Report ID number. The id number can be found on the CMS Validation Report
  2. Record the results from the report: A for accepted assessments and R for Rejected Assessments
  3. Click Save 

 

 

 

  1. Enter the date the Final Validation was received and click Save
