Action Code	Abbreviation	Action Type	When to Utilize
Actual Admission	AA	Admission	Utilize this action code if any of the following scenarios are applicable: Very first admission to facility. If the last discharge was Return Anticipated and resident did not return within 30 days of discharge. If the last discharge was Return Not Anticipated
ReAdmission	RA	Admission	Utilize this action code if the last discharge was Return Anticipated and resident returned within 30 days of discharge.
Deceased Date (Facility)	DE	Death	Utilize this action code if the resident died within the facility.
Deceased Date (Hospital)	DH	Death	Utilize this action code if the resident died at the Hospital after they were transferred.
Discharge AMA Date	DAMA	Discharge	Utilize this action code if the resident discharged from the facility Against Medical Advice
Discharge Date	DD	Discharge	Utilize this action code if a resident discharged from the facility either planned or unplanned.
On Line Census	OLC	Internal Transfer	EHR team ONLY when facilities are activated in PCC.
Payer Change	PC	Internal Transfer	Utilize this action code if the resident’s primary payor has changed.
Room Change	RC	Internal Transfer	Utilize this action code if the resident is moved to a different bed within the facility.
Transfer Out/Skip Day-Integrity	TOSKP	Internal Transfer	Utilize this action code if the resident is absent from their bed at the midnight census however their absence is less than 24-hours.
Leave of Absence/LOA	L	Leave	Utilize this action code when the resident leaves the facility at the start of the resident’s LOA
Transfer Out to Hospital	TO	Leave	Utilize this action code when a resident is transferred to acute care for evaluation.
Return from Leave/LOA	RL	Return from Leave	Utilize this action code when the resident returns to the facility at the end of the resident’s LOA.
Transfer In from Hospital	TI	Return from Leave	Utilize this action code when the resident returns from the hospital if it is less than 24 hours after they were transferred.
Room Reserve	RR	Room Reserve	Utilize this action code when the resident needs a bed hold for a hospital stay.
