Correcting Rejected MDS Records

If an MDS record is rejected by CMS, you must unlock and correct the record before exporting or submitting it again. You cannot correct a rejected MDS record through the modification process. 

Note

You cannot correct MDS records rejected as duplicates. If an MDS record is rejected as a duplicate, the record was successfully submitted to CMS. For manual MDS submissions, locate the Final Validation report for the original submission and mark the MDS record as Accepted. 

This process consists of multiple steps.

To unlock and mark an MDS record as Incomplete  

    • From the MDS Summary, click Lock, then click Unlock.


    • From the MDS tab, click unlock next to the MDS record, then click OK. Click edit to navigate to the MDS Summary.


  1. If the MDS record is comprehensive, you must uncomplete the Care Area Assessment Process.

    1. Click Care Plan Decision, then click Yes.

    2. Click Care Areas, then click  Yes.

  2. Click Complete MDS, then select Yes

To correct an MDS response  

  1. Click the MDS Section.

  2. Locate the MDS item to correct, then click Un-Sign Question.

  3. Correct the response.

  4. Click Sign, select the response(s), then click Sign

To complete and lock an MDS record

  1. Click Complete MDS, type your password, then click Sign

  2. If the MDS record is comprehensive, you must complete the Care Area Assessment Process. 

    1. Click Care Areas, type your password, then click Sign

    2. Click Care Plan Decision, type your password, then click Sign.  

  3. Click Lock, then click Lock. The MDS record is ready to be exported or submitted.
